How to apply

1. Select the relevant Application Form and Terms & Conditions.
2. Complete the form by clicking in the appropriate fields and entering the required information. You may also use the TAB key on your keyboard to navigate through the fields on the form. Alternatively you may print the form and complete using a ball point pen.
3. If you are transferring an ISA to Julian Hodge Bank, please complete the Cash ISA Transfer Form which includes a Transfer Authority Form.
4. Print the form and sign and date it using a ballpoint pen.
5. If you are a new customer, the Bank will undertake searches as necessary to establish your identity. If we are unable to positively establish your identity from these searches, we will request you to provide us with further proof of your identity and permanent address together with any further information we may require.
6. Complete your cheque made payable to 'Julian Hodge Bank Ltd' and one or more of the individuals named as account holders drawn on a UK account in your name. Cheques payable to joint parties may not be paid into an account held in a single name.
7. Send your completed Application Form, identification (if applicable) and cheque (if applicable) to:
